Role Profile Healthwise Facilitator 1. Role Summary The role is responsible for prescribing suitable physical activity and supporting behavior change for Medically Referred clients, including setting up and undertaking group and individual training sessions along with general day to day operation of the GP Referral programme including referral processing, accurate record keeping and data processing. The Exercise Referral Instructor is responsible to the Healthwise Coordinator / Health Manager 2. We use the analogy of the Cube because of the connectivity between the six faces (in terms of how they either impact on others and/or reflect on them). Each of these themes sets out long term ambitions; which are regularly reviewed and updated and are reflected in the responsibilities and accountabilities of this role (Section 3 below). 3. Responsibilities and Accountabilities Programming/Operational * To ensure the smooth day-to-day running of the Physical Activity Referral Scheme (PARS), maximising effectiveness of all available resources * Pre Screen referred patients in line with PARS protocols. * Maintain a central referral database for all PARS patients * Carry out health and lifestyle assessments with referral patients and develop safe and appropriate programmes of physical activity / exercise. * Develop and deliver group activity sessions or one-to-one sessions where appropriate. * Carry out re-assessments with patients in line with operating procedures * Act as first point of contact for patients referred onto the scheme * Ensure that the PARS is patient centred. * Ensure positive communication throughout the scheme. * Support and work closely with Leisure Centre Managers and operational teams to complement their operation. * Support the PARS in achieving its targets * Monitor all sessions delivered within the scheme in line with the schemes protocols. • Ensure that all equipment used in your PARS Centre is maintained and serviced to the correct standards. • Produce information to enable completion of monitoring reports for relevant agencies. • To provide individuals with evidence-based, relevant and culturally sensitive information regarding a healthy lifestyle. • Attend meetings as required. Development/Direction * Assist with the development of programmes and schemes that address physical inactivity Professional/People Accountabilities * Ensure that all necessary qualifications for the post are updated * Ensure continuous professional development * Provide cover for other members of the GP Referral team as required. Budgets/Finances * Responsibility for managing allocated budgets as determined within the project. General * To be flexible to the needs of the PARS and GLL * To carry out duties with due regard for Health and Safety Requirements and regulations, the Society’s Equal Opportunities Policies and Customer Care standards. * Keep up to date with developments in Healthy Living and the Leisure Industry in general * The post holder will have access to confidential information. The post holder must not at any time use the personal data held by the PARS for any purpose other than that which it was intended under the terms of the scheme or disclose any such data to a third party Other * Act in accordance with, and actively promotes all GLL policies and standards. * Undertake any other duties commensurate with the post’s level of responsibility or required particularly as a result of events outside of GLL’s business control (pandemics etc). * Work flexibly in line with the needs of the business at peak flow times, evenings and weekends. * Deliver critical incident management as and when required. 4. Key Working Relationships * Reports to the Healthwise Coordinator / Health Manager. * To constructively interact with different people in a variety of areas within the operational team, wider health teams and GLL support services. * Builds good working relationships with all GLL colleagues, working both proactively and collaboratively. * Builds positive and progressive relationships with local authority partners and local health teams as required. 5.
